In commemoration of the Public profession of faith in the Reformed Church

Beloved,
You come before the face of God and in the midst of His congregation to profess your faith. Therefore I ask of you to answer the following questions in all sincerity:
1. Do you believe in the only true God: Father, Son, and Holy Spirit, in whose Name you were baptised?
2. Do you believe and profess that, although you were conceived and born in sin and are still inclined towards all evil, you nevertheless are made righteous in Jesus Christ, adopted by the Father as His child and raised by the Holy Spirit to a new obedience?
3. Do you believe and profess that both the Old and the New Testaments are the Word of God, and that it is faithfully summarised in the Ecumenical Creeds and the Forms of Unity of the Reformed Churches?
4. Is it your intention, through the grace of God, to persevere in this teaching until the end of your life? To remain in covenant fellowship with God; to love Him with your whole heart and to serve Him in obedience to His Word; to deny this world and its desires; to seek your life and holiness outside yourself in Christ; and in so doing confirm this confession of yours with a God-fearing life?
5. Do you promise, with the whole communion of the church of Christ is now unlocked for you, to eagerly attend to the preaching of the Word and the administration of the sacraments as a living member? To willingly and with joy use your gifts for the needs and edification of other members, and to use them towards the enlargement of the kingdom of God. And do you promise to submit to the admonishment and discipline if you should err in life or teaching?

Where you now have here publicly professed your faith and accepted God’s covenant promise which is sealed in your baptism, we urge you to persevere herein.
In the Name of our Lord Jesus Christ, we welcome you into full communion with the people of God. May God, Himself, who called us to His eternal glory in Jesus Christ, after we have suffered a short while confirm, strengthen and hold you completely. To Him be all the glory and power eternally. Amen
